      <title>Certified Nurses Aide</title>
      <description><![CDATA[<p><strong>GENERAL RESPONSIBILITY:</strong></p><p>A Certified Nursing Assistant (CNA) is a non-professional worker who, under the direction and supervision of a Registered Nurse, carries out simple nursing functions. These include attending the personal needs of selected residents, procuring and returning equipment, running errands and assisting in the general maintenance of the nursing unit.</p><p><br></p><p><strong>DUTIES AND RESPONSIBILITIES:</strong></p><p>1. Gives hygienic care: bathing, assisting with cleaning teeth, back rubs, providing water to wash hands and face, nail care, combing hair.</p><p>2. Gives, removes and cleans bedpan.</p><p>3. Prepares residents for meals, assists residents to eat.</p><p>4. Assists the Nurses in the care of residents as directed.</p><p>5. Reports all observations concerning residents to a Team Leader.</p><p>6. Makes beds, occupied and unoccupied.</p><p>7. Assists patients in moving and turning, and getting in and out of bed, and walking as needed.</p><p>8. Keeps water pitchers clean and filled with fresh drinking water.</p><p>9. Assists in keeping department neat and clean by: cleaning bedside unit, cleaning nursing equipment, keeping service areas in order (utility room, linen room, and kitchen).</p><p>10. Feed residents as needed – report any difficulty in eating or change in appetite to nurse.</p><p>g. Remove all trays from rooms to be returned to the kitchen.</p><p>3. Get linen ready.</p><p>4. Daily baths and showers to assigned residents. Provide privacy as per regulations. Knock on doors before entering rooms.</p><p>a. Mouth care – teeth or dentures daily.</p><p>b. Back care – back rubs, skin care</p><p>c. Peri-care.</p><p>d. Report to nurse any unusual findings with resident.</p><p>e. If resident needs any treatments for decubitus (wound care etc.) the nurse will do these – tell her when resident is ready.</p><p>5. Residents out of bed. Check those who need to be ready for therapy.</p><p>a. All residents are to be out of bed - unless otherwise ordered by physician.</p><p>b. All residents are to be dressed appropriately (no holes/rips or torn clothing).</p><p>c. Residents should be taken to TV room, dining room, activities, physical therapy room or outdoors as tolerated.</p><p>d. Complete all restorative care as ordered.</p><p>6. Assist with admissions and discharges as necessary.</p><p>a. Introduce self. Obtain height and weight of resident.</p><p>b. Bring resident to room and assist to undress and unpack.</p><p>c. Fill in clothing list and admission sheet completely.</p><p>d. Escort all discharge residents out of building.</p><p>7. Make beds using covered barrels for dirty linen. No linens on floor.</p><p>8. Assist with lunches (as with breakfast). Encourage residents to eat in the Dining Room.</p><p>9. Report to dining room at 11:30. Prepare residents for lunch. Take assigned residents to the dining room. Ambulate to dining as ordered. Sanitize resident’s hands; offer clothing protectors, or assist resident eating in rooms as assigned. Monitor dining room as assigned.</p><p>a. Pass trays – assist as needed.</p><p>b. Feed residents as needed – report any difficulty in eating or change in appetite to nurse.</p><p>c. Assist residents eating in rooms as assigned; remove all trays from rooms for return to kitchen.</p><p><br></p><p>10. Take breaks and lunches as assigned. If one person goes late it holds the next person up.</p><p>11. Bring residents to activities in the afternoon.</p><p>12. Walk residents as per schedule and to and from dining if able.</p><p>13. Check residents every two hours and provide for toileting, restraint release, change of position tec.</p><p>14. All residents are to receive fresh water and nourishments per policy. Change water sets and clean ice chest per policy.</p><p>15. Review CNA instruction sheet for accuracy – notify nurse of changes as necessary.</p><p>16. Prepare residents for care conferences. Take them with you to conference as assigned.</p><p>17. Afternoon duties:</p><p>a. Special assignments-utility room, linen, closets, etc.</p><p>b. Make sure resident rooms are neat – bedside tables clean, bedpans and urinals clean and put away. Closet draws are tidy and neat, clothing on hangers.</p><p>c. Empty catheters.</p><p>d. Make all empty beds in your assignment and prepare room for new admission.</p><p>e. Complete all charting; flow sheets, restorative documentation etc. Notify nurse of any changes or problems.</p><p>f. All personal items in plastic bags labeled with resident names.</p><p>18.       <title>LICENSED PRACTICAL NURSE</title>
      <description><![CDATA[<p>The L.P.N. assists the Charge Nurse in his/her responsibility for total care within the nursing unit and management of the unit. Performs any and all professional nursing duties. May act as Charge Nurse of the unit with all attendant Charge Nurse authority and responsibility. Provides nursing care according to physician’s instructions and in conformance with state approved Florida Board of Nursing Practice, established standards and administrative policies which may include more than the specific responsibilities list below.</p><p><br></p><p>Minimum Qualifications</p><p><br></p><p><strong>I. Education:</strong></p><p><br></p><p>Graduate of a State Accredited School of Practical Nursing and currently licensed to practice as a Practical Nurse in the State of Florida.</p><p><br></p><p><strong>II. Check and administer medications and treatments per standards of Florida Nurse Practice Act and facility policy; order medications from pharmacy; maintain narcotic records; count narcotics with nurse from outgoing and incoming shifts.</p><p>2. Assist the Registered Nurse in charge in giving more complex treatments.</p><p>3. Maintain resident care plans.</p><p>4. Carry out the resident care plan set forth for each resident.</p><p>5. Maintain accurate records of nursing and medical care received by the resident.</p><p>6. Implement plans through tasks such as wound treatment, monitoring pulse, blood pressure and temperatures.</p><p>7. Resident education.</p><p>8. Assist the Charge Nurse in supervision and management of the unit and assumes the role of Charge Nurse as assigned.</p><p>9. Observe and record the condition of residents.</p><p>10. Encourage and assist residents to participate in activities which promote physical and emotional well being.</p><p>11. Respond quickly to emergencies.</p><p>12. Perform and specific duties assigned by the Registered Nurse or the Charge Nurse.</p><p>13. Organize staff and work loads</p><p>14. Documentation of the care of the resident.</p><p>15. Make ethical decisions related to consent and confidentiality.</p><p>16. In all cases nurses need to establish a professional relationship with residents, families and physicians and other nurses through the community.</p><p>17. Other duties as assigned.</p><p><br></p><p><strong>VI. Working Conditions:</strong></p><p><br></p><p>1. Sitting, standing, bending, lifting and moving intermittently during work hours.</p><p>2.       <title>UNIT MAMAGER  / CHARGE NURSE</title>
      <description><![CDATA[<p>POSITION SUMMARY: </p><p>The Unit Manager assumes 24-hour per day responsibility and accountability for the nursing care of all residents on an assigned unit. Supervises licensed nurses and other nursing personnel in the delivery of care and services according to each resident’s comprehensive resident assessment and plan of care. Manages unit in accord with Facility philosophy in compliance with standards of professional nursing practice and with all State laws and regulations. Utilizes resources in an efficient and economic manner to ensure that each resident achieves the highest practicable physical, mental and psycho-social well being. </p><p>MINIMUM QUALIFICATION STANDARDS: </p><p>Education: Graduate of an accredited school of nursing. </p><p>Certificate/Licenses: Current, active license as registered nurse in Florida. </p><p>Work Experience: Two years professional nursing experience providing direct care in long-term care facility with one year supervisory or management experience with direct care staff. </p><p>Freedom from illegal use of drugs, and freedom from use and effects of use of drugs and alcohol in the workplace. </p><p>Persons who have been found guilty by a court of law abusing, neglecting or mistreating individuals in a health care related setting are ineligible for employment in this position. </p><p>PERFORMANCE REQUIREMENTS: </p><p>Knowledge of nursing principles (geriatric, other specialty, general chronic care) and professional standards of nursing practice and ability to apply to resident specific circumstances. Participates in formal quality assessment and assurance activities as directed by the Quality Assurance Committee. </p><p>As the nurse with responsibility for the residents on the unit, conducts or coordinates with the interdisciplinary team resident assessment process including accurate completion of the state-approved Resident Assessment Instrument. </p><p>Participates as member of interdisciplinary team in development of plan of care for each resident that assists the resident to attain or maintain the highest practicable physical, mental and psycho-social well being. Plans of care are developed with the participation, to the extent practicable, and consent of the resident and/or resident’s interested family member, legal representative. Plans of care include measurable objectives and timetables and accommodate resident’s needs and preferences including choices about preferred intensity of medical treatment. </p><p>Attends interdisciplinary conferences to plan care. Elicits input from direct care staff on all three shifts and provides opportunities to attend and participate in interdisciplinary care conferences. </p><p>Supervises nursing staff in the implementation of plans of care to assist residents to attain or maintain highest practicable physical, ental and psycho-social well being. </p><p>Maintains ongoing communication with other disciplines to evaluate resident responses to interventions, to suggest alternative approaches when indicated and to revise and update plans of care to reflect progress toward goals. Facilitates interdepartmental communication to meet residents’ needs. </p><p>Informs the resident in advance about care and treatment and any changes to the plan of care. </p><p>Immediately informs the resident, consults with the resident’s physician and notifies the designated family member and/or the resident’s legal representative when there is an accident involving an injury which has potential for requiring physician intervention; a significant change in the resident’s physical, mental or psycho-social status; a need to alter treatment significantly or a decision to transfer or discharge the resident from the Facility. Promptly notifies the resident, designated family member and/or resident’s legal representative when there is a change in room or roommate. </p><p>Interacts with residents, family members, legal representatives, surrogates to answer questions, resolve concerns. Makes prompt attempts to resolve all resident grievances, seeking consultation or assistance from other departments or administration as appropriate. </p><p>Educates and develops unit nursing staff members by observing practice, interpreting facility policies and procedures, identifying learning needs and taking advantage of opportunities to teach on the unit. </p><p>Monitors unit nursing staff for correct, consistent application of infection control procedures. Consults with (Assistance Director of Nursing) on questions related to infection control. </p><p>Assists Unit Nurse with the assignment of nursing staff to ensure sufficient number of staff. Evaluates staffing patterns based on unit census, resident acuity, other resident needs. Adjusts staffing pattern accordingly. Communicates difficulty meeting resident needs and maintaining regulatory compliance promptly to Director of Nursing. </p><p>Adjusts own work schedule to maintain direct contact with and to evaluate performance of nursing staff on all three shifts. </p><p>Conducts performance evaluations of unit nursing staff based on demonstrated competency to perform essential functions of the job. Conducts skills observations of nurses aides. </p><p>Actively participates in long-term care survey process. Accompanies surveyors at all times while on unit. Provides or offers supplemental information to ensure accurate survey results. Responds to surveyor questions. Records actions of survey team members and reports to Director of Nursing at end of each survey day. Consults with Director of Nursing about undertaking corrections while survey is in progress. </p><p>Completes required documentation of care and services delivered during shift including subjective findings, objective symptoms, interventions and resident responses to interventions. Completes required documentation of special circumstances including accident/incident reports in compliance with State and Federal laws and regulations. </p><p>Reports relevant information to the Director of Nursing including, interdisciplinary team functioning and communication, regulatory compliance issues, resident risk factors, sudden changes in resident behavior, expressed dissatisfaction by resident or family, refusals of treatment, unexplained injuries, staff behavior (positive and negative), staffing needs, equipment and supply needs, opportunities to revise policies and procedures and to improve quality. </p><p>Monitors staff for compliance with OSHA mandates on workplace safety including hazard communication and blood borne pathogens. In cases of workplace exposure, provides immediate first-aide and refers employee to appropriate facility staff member for post-exposure follow-up. </p><p>Reports all injuries to self or others to supervisor immediately. </p><p>Takes job actions with staff members under appropriate circumstances. Applies facility policies about progressive discipline correctly. Removes staff member from unit if necessary to avoid endangerment to health or safety of residents or other staff members. Reports all job actions taken to Director of Nursing promptly. </p><p>Attends continuing professional education programs and in-service education. Applies information to job responsibilities. </p><p>Participates in fire and disaster drills. In the event of an emergency, carries out assigned duties to assure resident safety. </p><p>PHYSICAL, PSYCHOLOGICAL AND ERGONOMIC REQUIREMENTS: </p><p>Successful performance of essential functions can best be achieved through consistent application of current knowledge, use of good judgment, common sense, ability to establish and carry out priorities, effective use of interpersonal skills and ongoing communication with residents, staff, families, interdisciplinary team members, and government officials, including State surveyors and ombudsmen. </p><p>This job cannot be performed without exposure to the stresses associated with an intimate, 24-hour residential care environment that delivers care and services primarily to disable and cognitively impaired residents.
